What happened

Low-experience jumper (57 jumps) on a stability check jump with a jumpmaster from 12,500 ft, the fourth jump of the day. After separation the main was never deployed and no emergency procedure was carried out; video suggests the jumper was reaching for the main handle. The automatic activation device fired at 750 ft as designed, but the reserve container did not open. The Norwegian Air Sports Federation's investigation found that the clear window in the reserve flap of this container model most likely prevented the reserve from opening; the manufacturer found the fault in tests, 25 rigs were grounded and a fix was issued. Why the main was not deployed was not explained.

Lessons for safety

Practise emergency procedures until they are automatic, including in a horizontal (freefall) position. Equipment safety notices matter – check whether they apply to your rig.
