What happened
First AFF jump, from 14,000 ft. She did not respond to the altitude check. The instructor lost grip at about 4,700 ft and caught her again at 3,000 ft. The main pilot chute did not inflate in the burble. Both AADs fired; the instructor's main and the student's reserve deployed at the same time, and the main opening interfered with her reserve.
Lessons for safety
Students must deploy at the assigned altitude; instructors need a clear plan for a student who does not pull.
